Lumata Health works directly with your retina specialist to help you stay on track with your care plan, manage your condition confidently, and protect your vision for the long term.
About the Continuous Care Program
Managing a chronic eye condition can feel challenging. Our Continuous Care Program is designed to make it easier.
Through this program, eligible patients are paired with a dedicated Care Coach who works alongside your doctor to help ensure you receive the right care at the right time
Your Care Coach will check in regularly to answer questions, provide education, and help remove barriers that could stand in the way of your treatment plan. This service supports—but does not replace—your regular eye care. Your Eye Associates Northwest provider remains your primary contact for urgent or emergent needs.
How Your Care Coach Can Help
- Check in between visits to see how your vision and overall health are doing
- Clarify your doctor’s instructions and answer your questions
- Share helpful tips and educational resources for managing your condition
- Assist with locating affordable prescriptions, supplements, or vision-related items
- Help coordinate transportation or paperwork, when needed
